Achieve Beyond is a growing national agency focused on serving children with special needs by providing autism services, speech therapy, occupational therapy, physical therapy, and other related services. We are currently looking to hire a Bilingual Spanish Case Coordinator to join our growing team!

Responsibilites:

The Case Coordinator directly assists the Case Manager to maintain quality and quantity of caseload. Case coordinator is part of the EI Case Management team who serves as a liaison between service coordinators, EIODs, and providers.

  • Follow up on start dates twice a week
  • Follow up on cases requiring prescriptions/feeding clearances (missing, expired, expiring within 30 days)
  • Upload all Rx and reports to NYEIS system to child integrated case.
  • Follow up on cases requiring speech recommendations
  • Request and send out progress reports. Ensure all information on reports received is correct, make note of children that are no longer eligible/have a low percentage of delay and take appropriate action.
  • Maintains overdue progress reports to less than 5 (6 + 12 month reviews)
  • Assist CM with processing approved IFSP reviews (update database, send reviews to therapists, document new start dates, address worksheet errors)
  • Conduct parent surveys
  • Follow-up on PINS
  • Follow-up with Service Coordinators as needed.
  • Provide support with closing cases.
  • Maintain positive and professional relationship with all providers, EIODs, service coordinators, and managers
  • Provide coverage of case manager and receptionist phone during lunch breaks and/or office absence

Requirements:

Education and/or Work Experience Requirements:

    • Associates Degree in Human Services or related field preferred
    • 1-2 Years of experience working in an office or related field preferred, knowledge of excel

Other Requirements:

    • LANGUAGE SKILLS: Bilingual Spanish Preferred
    • Excellent customer service skills
    • Knowledge in Microsoft Office – particularly excel, outlook and word
    • Strong typing skills
    • Be able to multi-task under time constraints
    • Attention to details to ensure accuracy of information
    • Able to effectively communicate
